OKHPR attempts to send a candidate survey to each statewide candidate. Candidates who file for an election using their email address will receive a Parental Rights Survey from OKHPR via email. If you did not provide your email address when filing, or if the email was not received you may use the following form to request a survey. Surveys will be sent out after the election filing. Requesting a survey early will ensure you are sent the survey first.
