We are providing two templates of a religious exemption that was written by a minister on our OKHPR minister list. One is general for refusing all vaccines moving forward, as it contains language regarding aborted fetal tissue and animal parts or DNA. The other is for refusing covid vaccine only, as it only contains language regarding aborted fetal tissue.
