* UPDATE! Thanks to our new State Superintendent, Ryan Walters, the State Department of Education has created THEIR OWN reporting system here! Please report the non-compliance with the State Department of Education FIRST, then feel free to also report this violation to OKHPR! *

Please use this form to report Pre-K-12th grade schools, higher education, and career tech schools that are not operating in compliance with SB 658.

This law was passed May of 2021 and goes into effect July 1st 2021.

The bill specifies:

  • Mask mandates can only be issued by school boards, only under a state of emergency, and must be reviewed at each meeting.
  • Mask mandates not allowed for unvaccinated students only. No discrimination!
  • Schools must tell parents that they can opt-out of vaccination mandates using exemptions if parents so choose. Exemption information must be provided on the school's website and in any publication that is released by the school which mentions vaccination requirements.
  • Vaccine passports, otherwise known as, “papers, please” are not allowed in any level of education.
  • No C-19 vaccine mandate for Pre-K-12 and Higher Education, with the exception of some medical programs that must see patients.

For more information please read the full bill here, or watch an FAQ with the authors of the bill here.

OKHPR has provided a letter that can be sent to schools that are violating SB 658. You can find it here.

These reports will be kept completely anonymous. Contact information will only be used by the OKHPR team to gather more information if needed.
